YWCA ID Clinic

We are offering an Identification Clinic on Friday February 19th, 2016 10 am – 4pm. 

This clinic will offer client support in applying for health cards, birth certificates and SGI photo id.

There will be staff including Cherry Insurance, SGI identification issuer, to assist clients with their documentation and applications.  Please note Affinity Credit Union is sponsoring this clinic and will pay for the costs of obtaining identification (birth certificates and SGI Photo id).  Clients must bring documentation - – any documentation they may have will assist the application process.

Examples of appropriate documents include:

• treaty cards
• health cards
• income tax documents
• drivers licence
• employment confirmation
• rental lease
• bank statements
• utility bills
• government issued identification
• court order
• social assistance benefit confirmation
• library card
• band member verification
